inner ear problem

My cousin has suffered for the last 23 years with a bizarre complaint that
is centered
in her inner ear. What happens is that anytime in the day (more often late
in the day or sometimes at
night) her ear will suddenly produce a banging in her head. (Lately both
ears rather than just one).
She does get a little forewarning of the impending attack which may last 30
mins to an hour or more,
then just as suddenly the attack stops and all will be normal. As much as
she would like to, she can do nothing to hasten the end of the attack like
resting. The attacks are tending to get longer and more intense.

During the attack her speech is affected causing her words to be a little
hard to be pronounced and she is quite distressed. She has been tested a few
times, including two MRI scans, but no-one in the medical profession can
tell her what is wrong let alone any useful treatment. While undergoing
tests she has hoped for and tried to cause an attack to facilitate
diagnosis, but never managed to do so. She cannot say what actually triggers
an attack. She had wondered if it was her body temperature, or room
temperature, or noise pitch or sudden loud noises. Or maybe just general
tiredness that triggers an attack. There seems to be no common factor.

As the medics cannot diagnose what is wrong she is not taking any medication
for this condition. except for something called SERC which I believe is for
control of Ménière's Disease but that does not ease the problem. She has
tinnitus but thinks that maybe a separate issue altogether.

If anyone reading this post and has even the slightest idea of what the
trouble is and its remedy, I would most certainly appreciate a reply, which
I will pass onto my cousin.

Your mentioning that she has forewarning of the events might suggest
migraine ... atypical migraine or migraine equivalent, with the
"forewarning" being some kind of aura. Do the "banging noises"
correspond with her pulse? The difficulty speaking is also consistent
with migraine.

There are new drugs one can take daily to prevent migraines, if that's
what it turns out to be.

No the banging noises do NOT correspond to her pulse. And she rarely has
headaches anyway.

Migraine equivalents do not have the headache. They can include aura
and neurological symptoms such as difficulty speaking.
