well, DH has switched from humulog 75/25 to a fast acting humulog and
lantos. Unfortunately he still can't get his bg readings under 150 most
of the time. I'm not sure exactly how much of each he's taking, but it
is a lot more than our doctor suggested he start with.
So the good news is, since we are about an hour from Boston, I made an
appointment for him at the Joslin Clinic. Our doctor told him he should
go there many years ago, and he finally admits it's past time to go.
I'll tell you, armed with all the great information and support I have
found here, I feel we are more than ready to tackle this challenge.

Spouses, parents, grandparents............it isn't like they are 6 years
old, and you can just carry them to the car, and make them see whatever
doctor.
Mom resisted going to a nephrologist for her kidneys for the first 6 or 7
months that she lived with me. Her primary doc took things slowly, and kept
an eye on her labs. Unfortunately, she suffered an uncommon condition after
a surgery that caused her kidneys to fail rapidly.......she almost died.
When Mom went for her 30 follow-up exam, SHE asked the doc to get her an
appointment with the nephrologist.
Sometimes, the heart wrenching truth is that it takes something dramatic,
and sometimes life threatening to occur before we are ready to set aside our
fears and denial.
I don't know what the full story is with your dear hubby, but bg that won't
go below 150 can be spooky and frustrating.

oh thank goodness he's off that terrible premix sh** (now ask me what i
really think of it lol)
good luck at the Joslin.......hope all goes well and they are able to
titrate him quickly......... perhaps an e-mail contact with them on an
ongoing basis will assist in this endeavour???

You somehow figure out your carb to Humalog ratio. Then you count the
carbs in a meal so that you can shoot enough Humalog to "cover" the carbs.
I shoot 1 unit Humalog for about 12 -15 gram of carb but I'm a Type 1, i.e.
a non-Insulin Resistance fellow.
An Insulin Resistance person might expect to shoot about 1 unit for each 7
gram of carb.
If I have high sugar before a meal, I shoot my Humalog, then wait a bit to
give it a chance to start working and driving my bG down before I eat.
That only works if you have reasonable control over "when" the food will
be in front of you though I find that I can safely shoot 30 minutes early
with almost no problem (that works in restaurants) , and 60 minutes early
if I am much too high (but that only works at home; it's dangerous if you
don't have control over the food! )
My local hospital has two types of Diabetic Training courses.
a. The basic Type 2 Diabetes training (10 classroom hours)
b. Insulin-Users training (9 classroom hours)
The insurance companies recognize the difference between the courses. The
insulance enforces a waiting period between "repeats" but taking one, then
the other is not a "repeat".

Did his dr start him on 20-30u? That seems MIGHTY high, usually they
start a resistant person off at 10u and move upward over a few weeks or
months.
It's possible he's taking WAY too much insulin and having liver dumps to
cover it naturally. Not everyone has hypos when they overshoot. I don't,
I have a very active liver, from the cgms i've had, I can drop to 50 and
not know it, liver kicks in and i'm at 130 or higher in no time, Before I
would test and realize i'm high, then correct again, till I realized what
was happening.
personal experience tells me, I'd reduce the insulin and see if I get better
results first, if not, then yep, he's prolly very resistant. But you won't
know
till you experiement some.
