Understanding osteoarthritis of the knee--causes and effects.

Case Western Reserve University School of Medicine in Cleveland, Ohio,
USA. Osteoarthritis of the knee is common, increasing with age in both
women and men, but is generally more prevalent in women following the
fourth decade. Osteoarthritis may be primary/idiopathic or secondary
as a consequence of trauma, surgery, infection, or another disease
process. Normal articular cartilage is composed of an extracellular
matrix and chondrocytes. This matrix contains water, collagen fibers,
and proteoglycan macromolecules cross-linked into an integrated
network with hyaluronic acid. Osteoarthritis represents an imbalance
in the destructive and synthetic processes of the cartilage that leads
to erosion of the cartilage. In addition, there is a decreased
concentration and viscosity of the synovial fluid in osteoarthritic
patients, and this may decrease the lubricating and cushioning
properties of the joint. There is also an underlying inflammation of
the synovium, as well as damage or reactive changes in the subchondral
bone. The entire process is thought to involve a complex interaction
of cells and soluble mediators such as cytokines, growth factors,
inflammatory mediators, metalloproteinases, and chondrodegradative
enzymes. Understanding the biochemical and molecular changes that
occur in the joint is requisite to the development of treatments for
osteoarthritis of the knee that address both the symptoms of pain and
loss of mobility as well as the underlying disease progression. The
clinical goal of the management of osteoarthritis should be to treat
not only the symptoms of the disease, such as pain and decreased
mobility, but also the underlying pathology of the degenerative
process.

People with haemophilia experience a progressive deterioration of
their functional health status. Regular clinical assessment of
functional health status provides insight into their process of
disablement. As such, the development of a core-set of measurement
tools is warranted. The aim of this study was to gather data to
prepare a (preliminary) core set of clinically relevant and feasible
instruments to assess the functional health status of adults with
haemophilia, and to indicate their psychometric qualities. Therefore,
clinimetric instruments frequently used in two haemophilia-resembling
diseases (Rheumatoid Arthritis and Osteoarthritis) were reviewed from
the literature. An extensive search in Medline yielded 13 relevant
review articles, incorporating a total of 182 instruments, of which 40
were appropriate for haemophilia. Of these 40 instruments 3 measure
body structures, 13 body functions, 19 activities (of which 5 are
performance based and 14 self-report based), and 3 measure
participation. This classification is based on the International
Classification of Functioning, Disability and Health. Detailed
information regarding the psychometrics (reliability, validity and
responsiveness) of four instruments is described fully in the
literature, whereas the psychometrics of the majority of the other
instruments are only partly described. The results of this literature
study may contribute to the formation of a (preliminary) core set of
clinimetric instruments to assess the functional health status of
adults with haemophilia. Decisions on the final core set should be
held within the Musculoskeletal Committee of the World Federation of
Haemophilia.

Synovial membrane and fluid morphologic alterations in early
rheumatoid arthritis: microvascular injury and virus-like particles

Eight patients have been studied during the first 6 weeks of
rheumatoid synovitis. All of them exhibited microvascular injury,
which was manifested by gaps between endothelial cells, vascular
occlusion, erythrocyte extravasation, or endothelial cell injury. In
four patients, a variety of virus-like particles were found associated
with the endothelium or perivascular cells. In two cases, particles
were seen in electron-dense deposits in vessel walls. Lymphocytes and
PMN infiltrated the synovial membranes, but plasma cells were
uncommon. Evidence of phagocytosis was prominent in synovial lining
cells and other large mononuclear cells, but not in PMN. These
observations are consistent with injury to synovium and, specifically,
synovial vessels as an early stage in RA synovitis. The virus-like
particles require further investigation, because nonviral cell
components remain very difficult to distinguish in electron microscopy
tissue sections.

Of course now you'll have to attack me for daring to question your
