Written Contract for CNA?

Hello again folks, It's coming up on tax time and I have been told by the NC state employment security dept that we need a written contract between us and our CNA to prove to the state that she is indeed a contractor. Both she and I have understood this since she started this summer, but we don't know where to get a contract form to use as a guide. I would appreciate any info from anyone who has a contract with a CNA or for similar home-help personnel.

I have been taking OTC Naproxen Sodium (Naproxsyn) 220 mg. tabs for about 2 weeks. I take 1 tab twice a day and it "seems" to be quite effective for the usual arthritic aches & pains. Also, it doesn't cause me stomach problems like an "effective" dose of ibuprofen does. Anyone been taking Naproxen Sodium for a long period of time? Thanks

did the mri last thursday, got a call from the ortho specialst/ surgeon. he said it's an MT. his suggestion was that i should get it fixed soon because he wanted to avoid the possibility of my knee buckling and thus making things worse. it sounded like the urgency of the surgery was preventitive. thing is, i'm going on vacation two days after the scheduled surgery, so i REALLY want to put it off. ie,,,i don't want complications screwing up the vacation. so i have some questions.

Several months ago my ENT gave me an Rx for 10 mg. Prednisone tabs as well as an antibiotic to treat water retention in my ears. It worked well but I also was aware that my usual aches and pains (osteoarthritis) virtually vanished during the 2 week regimen on Prednisone and for months thereafter. It was amazing. I take NSAIDS like Ibuprofen (600 mgs.) and find them somewhat helpful but they do cause gastric irritation. I find acetaminophen to be essentially useless.

surgery for knee arthritis?

I have been having bad knee problems for a number of years, slowly worsening. Anyway, x-rays + MRI reveal arthritic changes (more inflammatory than osteo wear and tear) as well as torn meniscus cartilege. I will probably need scope "clean out" surgery. I know this surgery is excellent for the cartilege problem but I suspect much of my pain is the arthritis (but we don't really know).
